Managing Questionnaires
To manage questionnaires for ERA Care:
Login to the ERA Admin hub as an admin user
Find the ERA Care Questionnaire card, click Create or View. Note that the Create button is not available if a Questionnaire already exists
Create a new Questionnaire if one does not already exist. You can only create one for each organisation
Click Edit to make changes to your existing Questionnaire
Categories
Questionnaires can have multiple categories. Categories seperate the questionnaire into different sections, for example for different areas of follow-up care. Each category is scored independently and contains the following:
Category name (e.g. Your Birth Experience)
Low threshold
Message (the message that is sent to the patient if they trigger the low threshold score)
Score (the score required to trigger this threshold)
Optional referral email (an email address that will receive a referral if the patient triggers this threshold with details about the patient and their support needs)
Medium threshold
Message (the message that is sent to the patient if they trigger the medium threshold score)
Score (the score required to trigger this threshold)
Optional referral email (an email address that will receive a referral if the patient triggers this threshold with details about the patient and their support needs)
High threshold
Message (the message that is sent to the patient if they trigger the high threshold score)
Score (the score required to trigger this threshold)
Optional referral email (an email address that will receive a referral if the patient triggers this threshold with details about the patient and their support needs)
Category questions (these are the questions and available answers that are displayed to the patient when they complete the questionnaire)
Questions
Questions can be created inside categories. You can have as many questions as you need. Each question can have its own custom answers (usually Yes or No) and each answer must be assigned a score, which corresponds to the calculation of the total score for each category. This score is used to determine the threshold for the patient in each category (low, medium or high), which in turn determines which messages the patient receives, and if a referral is required.
