Patient Questionnaire

After 4 weeks from the date of the event, the patient will receive a link to the Questionnaire with a message which contains an overview of the service. The patient can only complete the Questionnaire once. If the patient does not complete the questionnaire, they will be sent a reminder after 48 hours, and again after another 48 hours, after which no more reminders will be sent. The patient can opt out of communications at any time by clicking the unsubscribe link which appears in the messages.
